Mycetoma is caused by the subcutaneous inoculation of filamentous fungi or aerobic filamentous bacteria that form grains in the tissue. The purpose of this study is to describe the epidemiologic, clinic, laboratory, and therapeutic characteristics of patients with mycetoma at the Oswaldo Cruz Foundation in Rio de Janeiro, Brazil, between 1991 and 2014. Twenty-one cases of mycetoma were included in the study. There was a predominance of male patients (1.3:1) and the average patient age was 46 years. The majority of the cases were from the Southeast region of Brazil and the feet were the most affected anatomical region (80.95%). Eumycetoma prevailed over actinomycetoma (61.9% and 38.1% respectively). Eumycetoma patients had positive cultures in 8 of 13 cases, with isolation of Scedosporium apiospermum species complex (n = 3), Madurella mycetomatis (n = 2) and Acremonium spp. (n = 1). Two cases presented sterile mycelium and five were negative. Six of 8 actinomycetoma cases had cultures that were identified as Nocardia spp. (n = 3), Nocardia brasiliensis (n = 2), and Nocardia asteroides (n = 1). Since December of 1983, clinical trial was undertaken on the prophylactic use of amikacin sulfate (AMK) and metronidazole (MET) in colonic cancer surgery. Daily dose of AMK 1,600 mg and MET 1,000 mg was administered orally in 4 divided doses to 17 patients with colonic cancer for 3 days prior to operation. As a result, wound infection was recognized in only one of 17 cases treated (5.9%). An examination was also performed on bacterial flora in the feces. After the administration of AMK and MET, E. coli and Klebsiella were remarkably decreased, however, E. faecalis and Candida remained unchanged. P. aeruginosa and Bacteroides detected in a few cases were also reduced. These bacteria were restored to the pretreatment level in 1 or 2 weeks after operation.

The biological activity of recombinant interleukin-2 in intravenous admixtures containing gentamicin sulfate, tobramycin sulfate, amikacin sulfate, ticarcillin disodium, piperacillin sodium, morphine sulfate, or total parenteral nutrient (TPN) solution was investigated. Recombinant interleukin-2 in a formulation containing human serum albumin was diluted to a therapeutic dose and mixed with the other drugs in test tubes to simulate mixing distal to the Y-site of an i.v. administration set. All test tubes were visually observed for precipitates. Triplicate test samples of recombinant interleukin-2 and interleukin-2 with drug were added to microplate wells containing lymphocyte tissue culture media (an interleukin-2-dependent murine cytotoxic T cell line, CTLL-2) using serial dilutions. The microplates were incubated then pulsed with mouse lymphocyte media containing tritiated thymidine (3H-thymidine). The amount of 3H-thymidine incorporated into lymphocyte DNA was measured using a beta counter. The average counts per minute (cpm) of a sample was analyzed as a function of the logarithm of sample dilution; one interleukin-2 unit was defined as 50% of the maximum cpm measured in the samples.
